Attention Parents!

Beginning this school year, the Student Medical Information & Parent Consent Form will only be available online through the Focus Parent Portal. This replaces the paper medical card that was traditionally sent home on the first day of school.

Please complete this form by the end of the first week of school. Students cannot receive services from the school clinic without a completed form on file.

⚠️ Beginning Wednesday, August 12, parents will be required to complete this form before they can access the Focus Parent Portal. We encourage families to complete the form as soon as possible to avoid any interruption in portal access.

How to Complete the Form in the Focus Parent Portal

📱 Using the Focus App

  1. Log in to the Focus Parent Portal app.

  2. Tap the Forms icon in the bottom right corner.

  3. Select the green "Not Yet Started" button.

  4. Complete and submit the Student Medical Information & Parent Consent Form.

💻 Using a Computer

  1. Log in to your Focus Parent Portal account.

  2. Click on the Student's name.

  3. Click Forms Summary on the left-hand menu.

  4. Select Student Medical Information & Parent Consent under Available/Pending Forms.

  5. Complete and submit the form.

The form is available now.
