The Department of Health in Okaloosa County is now offering school physicals during this year’s back‑to‑school rush!
Available from July 27 through August 20.
Services include $45 school physicals and a $15 immunization record review/transcription (capped at $45 for families with more than three children) to issue a DH 680 form.
All new KG students must have their Florida Immunization Form 680 and current school physical turned in before they start school; all other grades will have 30 school days to get these items.
