How To Enroll

In order to enroll, you will need to go directly to the school where you wish to enroll, bringing proof of your child's age, a medical examination (physical), contact information in case of emergency and your address and Social Security number.

To be enrolled in kindergarten, a child must have reached the age of five on or before September 1st. In kindergarten and in certain other grades, you will also need to show proof of immunizations. Immunizations which are necessary for school are provided free of charge at the Health Department locations in Crestview and Fort Walton Beach. Universal pre-kindergarten for 4-year-olds is provided through the Early Learning Coalition, not affiliated with the School District.                            

To complete the registration process, parents must present proof of the child’s age (birth certificate), a Florida certificate of immunization, a social security card, proof of residency (utility bill or lease agreement), and a school physical dated no later than twelve months prior to the first day of school.
