Welcome to Incoming 6th Grade Registration for the 2021-2022 School Year!

Our teachers and staff are so excited to meet our newest Panthers!!

Due to current restrictions, our “Prepping Our Panthers” registration process
will be virtual this year. We have prepared a registration information video to
help you in becoming a Panther.

*Please make sure your current school has all your up-to-date contact information
(contact numbers, address, email, etc.)

Do you need to change your child's transportation? Please use the note template HERE. All transportation

changes must be turned in to the Discipline Office by 9:00am. We cannot accept

transportation changes over the phone, due to safety and security reasons. Thank you!

The following must be included:

  1. Student’s First and Last Name
  2. Bus number student will be riding; 
  3. My Child will ride bus # ___ to (Complete Address - Street Number and Street Name)
  4. Identify if this will be a permanent change (If appropriate)
  5. Parent Phone Number - Required to verify change.
  6. Parent's or Guardian's Printed Name and Parent's or Guardian's Signature 


Please note: Prescription medication must have a current prescription on it, with instructions on how 

the medication is to be dispensed. Over the counter medications must be age appropriate and not be expired.


  • 7th Grade Immunization Requirement

Tdap immunizations are required for all 7th grade students. 
Any 7th  grade student that does not have an updated 
Florida Form 680 on file at Davidson MS 
will NOT be 
allowed to start school. Forms can be dropped off to the
