The guidance department at Davidson provides services in educational planning and placement, coordinating assessments, interpreting assessment results, coordinating occupational and career information, providing solution-focused brief counseling, assisting with outside referrals as needed, and supporting all students, parents, and teachers. Students and parents are encouraged to make appointments. Students may stop by the Guidance Office for an appointment to see their counselor.

There are many different reasons that a student may need to see the counselor.

  • To learn positive coping skills
  • Problems at home such as family changes, or a death in the family
  • Academic guidance -To get help with organizational skills, course selection, study skills, test taking skills, or time management
  • Learning coping strategies for anxiety
  • Getting help with making and keeping friends
  • Help with making responsible choices – Problem solving
  • Because they just need to talk to someone

Student Registration for 2021 - 2022 School Year

To register your student, please visit the guidance office to pick up registration paperwork and course selection materials. To determine if Davidson Middle School is your zoned school, please use the  School Site Locator.

Documentation needed for registration:

When registering your student at Davidson Middle School, you will be responsible for providing the following information:

  • Birth Certificate
  • Social Security Card (Optional)
  • School Physical (within the last year)
  • Immunization form from physician/Health Department verifying up-to-date immunizations (must be on Florida Form 680)
    • Students are required to have their TdaP prior to starting 7th grade
  • Name of last school attended, address, phone number, fax number
  • Proof of address (2 sources required)

            Required

  • Deed, mortgage/HUD statement, monthly mortgage statement, or residential rental/lease agreement covering the current year

            Second Source

  • Auto or homeowner’s insurance policy dated within the last 30 days.
  • Parent/legal guardian driver’s license ID card with the stated current address as that of the registering address
  • Mail from employment and/or financial institutions, including checking, savings, property tax record, credit card statements or investment account statements dated within the last 30 days
  • Mail from Federal, State, County, or City government agencies (including city and county agencies) dated within the last 30 days
  • A letter from a homeless shelter, transitional service provider, or a half-way house verifying that the parent/guardian resides at the given address, dated within the last 30 days (No other documentation required)
  • Most recent report card and/or school records
  • Copy of most recent IEP or 504 plan (if applicable)

If you are enrolling a student who is NOT your birth child, you must provide proof of:

  • Legal custody, court ordered, Children & Families placement, foster care provider, or agency documentation
  • In loco parentis as stipulated by Okaloosa County School Board policy

 

All required information must be on record at Davidson Middle School before your student will be enrolled and/or receive a schedule of classes.

 

Required vaccination for 7th graders

Students entering, attending, or transferring to seventh grade in Florida schools are required to have:

One dose of tetanus-diphtheria-pertussis vaccine (Tdap).

When your student has received his/her immunizations, please bring in the immunization certificate on Florida Form 680 to the front office prior to the first day of school.

Students will not be able to receive their schedule or start school without proper immunization documentation.

Military and Family Life Counseling

Davidson is pleased to announce the continuance of the Military & Family Life Counseling (MFLC) Program at our school this year.  The Child & Youth Behavioral Counselor (CYB), offers short-term, non-medical counseling support to assist military youth, parents, family members and staff. 

Typical issues the counselor addresses include:

·         school adjustment

·         stress management and coping skills

·         anger management/conflict resolution

·         deployment and reintegration issues

·         parenting, behavioral concerns

·         fear, grief, and loss

The MFLC provides support through engaging in activities with the youth, being available to parents and staff to discuss interactions with youth, and facilitating psycho-educational groups.
