Okaloosa County School District strongly recommends a valid and complete neurocognitive baseline test be established for each high school and middle school student-athlete prior to “in-season” sport participation.
A baseline test is considered valid for two years. Student-athletes who have never been tested, as well as those last tested prior to June 2016, need to be tested this year. Baseline tests are recommended for all sports’ student-athletes (including golf, cross country, swimming, cheerleading, etc.). Please refer to the Baseline Testing Schedule to see what day testing will be done at your school.
If a student-athlete has to miss his or her school's baseline testing date for a legitimate reason then they are allowed to attend one of the other school's testing sessions, however they must:
- Arrive only during the pre-established “visiting” student-athlete arrival time (11:00am – 11:30am)
- Make sure to clearly identify themselves as a student of a different school so their data is input into the system under the correct school
- Have a signed consent form
All student-athletes must have a SIGNED consent form to participate in baseline testing. Please download, print, and complete the Concussion Baseline Testing Consent form and bring it with you to testing. It is REQUIRED!! Also, wear any eyeglasses or contacts you usually wear to your testing session.
