Student Medication Dispersal


Before any medication may be left at the school for administration, parent permission and instructions must be obtained. The permission form (MIS Form 5183) authorizes school personnel to assist in the administration of the medication. This form must accompany the medicine. For your convenience MIS 5183 may be accessed by clicking on the link below.  The medicine must come in the original labeled container. Changes in the dosage or time cannot be made without a physician's written permission.

The health clinic aide will dispense medication. Students who take medicine on a regular basis (e.g., Ritalin) should be sent to the clinic at their designated time for their daily medication.

When it is necessary for students to keep medication with them at all times (e.g., asthma inhalers, emergency kits for bee stings, enzymes, etc.) a physician's written order stating such must be obtained and kept with the official MIS permission form for the student.

All medications must be brought to school by the parent and the permission form filled out and signed before we can give your child the medication.

According to school medication policy the first dosage of any new medication shall NOT be administered during school hours due to the possibility of an allergic reaction. Additionally, a student should not attend school if the first dose of medication is administered just prior to sending the child to school unless the parent stays on campus at all times the first day the medication is given and assumes responsibility if the child has a reaction. 


**Note: We can't give your child any medication if the child brings the medication and the form.  It must be brought by the parent.
