
In order to enroll, you will need to go directly to the school where you wish to enroll, bringing proof of your child's age, a medical examination (physical), contact information in case of emergency and your address and Social Security number.
To be enrolled in kindergarten, a child must have reached the age of five on or before September 1. In kindergarten and in certain other grades, you will also need to show proof of immunizations. Immunizations which are necessary for school are provided free of charge at the Health Department locations in Crestview and Fort Walton Beach. Contact the Health Department for more information.
